New Patient Information
Oasis Psychiatry is currently accepting new patients. Appointment requests are carefully reviewed by our providers on a case-by-case basis. Please note that certain limitations may apply regarding who can be seen in our practice.

Processing new patient requests may take up to a week or longer during busy periods. We kindly ask for your patience and request that you refrain from submitting multiple requests, as this may delay our ability to respond to everyone in a timely manner.

Existing Patient Information
To schedule a follow-up appointment, you can:

• Call us during phone hours to discuss scheduling options.

Initial Evaluation Process

1. Complete Intake and other forms
   • Fill out all required fields of Intake form, including insurance information if you have active coverage. This helps us verify your outpatient mental health benefits.
2. Provider Review:
   • The form will be reviewed by the specified provider.
3. Next Steps Upon Approval:
   • You will receive two emails:
     1. New Patient Forms via Tebra.
     2. Instructions to set up your patient portal and complete portal documents.
   • All forms and documents must be completed before scheduling your appointment.
4. Scheduling Your Appointment:
   • We will contact you to schedule.
5. If We Are Unable to Accommodate Your Request:
   • We will notify you via email or phone.
6. Follow-Up:
   • If you do not hear from us within 3-4 days after submitting the forms, please call or email us for assistance.
